These items must be completed and submitted prior to beginning services.

For New Contract Personnel Only

Complete and submit this packet of information to your company/agency and include all the following TOGETHER at the same time (items 1a, 1b, and 1c).
Note: your employer must sign the first page).

Provide proof of TB clearance by either a
TB Risk Assessment, TB test or exam (from within the past 60 days) or a chest x-ray (no more than four years old). See here for the

Provide a copy of your current License/Credential/Permit (if required by your position)

Provide a clear, color photo of yourself (from the waist up) for your school
ID Badge

Complete the Mandated Reporter Training for K-Post Secondary Sites Only (once we receive the above items, you will get an email containing an unique link to the course, please do not send us your certificate)
Read our
Compliance Handbook and sign our
Technology Agreement

Training for Early Learning Sites Only (This is in addition to the Mandated Reporter Training above)

Health Screening Report

Submit your Immunizations record for pertussis (whooping cough), measles and the annual flu vaccination (proof of a flu shot is required between August 1 and December 1s) or complete this
Immunization Verification/Waiver Form (must be signed by a health care provider).
After you have submitted your complete contractor’s packet to your company/agency's hiring department, they will forward it for you to the appropriate staff at Santa Clara County Office of Education (SCCOE). You will then be contacted to schedule a Live Scan background check fingerprint appointment at our main facility at 1290 Ridder Park Drive, San Jose 95131 in the Human Resources department on the second floor. Badges are issued once all requirements above have been met.
We fingerprint Tuesday through Thursday from 8:30am to 3:30pm. Appointments are every 15 minutes. The cost will vary. Check out our
Live Scan schedule.
If you are one of our contract companies and wish to create a billing account for Live Scan, please contact
HRprogramsupport@sccoe.org or 408-453-4362. Leave your name and phone number with the name of the company requesting to set up the account and someone will contact you to obtain the required information.

- You may retrieve another copy of your final certificate at any time by logging back into this account
