• Annual Request for Vacation Pay-off Form

  Payroll must receive the Annual Request for Vacation Pay-off Form with each line completed and with an original signature by May 15th. Only permanent Aides, OSS or OTBS employees on pro-rated calendars are eligible to complete this form.
