Eligibility and Enrollment
Dear Parent/Guardian:
Thank you for your interest in the Children’s Services
Department, Head Start Program. Our program offers educational and comprehensive
services in various locations throughout Santa Clara and San Benito Counties for
children 3 to 4 years of age. The Head Start Program provides double session
(half day) and full day preschool services. Parents, who are employed full time
or enrolled full time in school or a training program, may be eligible for full
day preschool services.
INCOME QUALIFICATIONS
Children will be accepted into the Head Start Program if they meet the 2009 Federal Head Start Income
Eligibility Guidelines listed below.

For family units with more than 8 members, add $3,740 for each additional
member.
THE FOLLOWING ARE ELIGIBLE REGARDLESS OF INCOME:
- CHILDREN WITH DISABILITIES (Child with current IEP or IFSP)
A Child (ren) who has a current Individual Education Plan (IEP) or
Individual Family Service Plan (IFSP) is automatically eligible for the
program. This means they are receiving special education services such as
speech therapy, resource specialist, or special education preschool. For
your application to be complete, proof of income and a copy of the
most recent IEP or IFSP are required. With a current IEP or IFSP, you do not
need to meet the income
guidelines listed above; however, lowest income is priority.
- CHILDREN IN FOSTER CARE
Children in foster care are automatically eligible for the program and must
provide proof of legal custody and income but do not need to meet the above
income guidelines to qualify.
- HOMELESS FAMILIES
- CHILDREN WHO’S PARENTS ARE ENROLLED IN CalWORKs
- Families receiving TANF benefits under the CalWORKs Program are
automatically eligible and must provide a copy of the Notice of
Action.
- CHILDREN WHO RECEIVE SUPPLEMENTAL SECURITY INCOME BENEFITS (SSI)
If your child receives SSI, he/she is automatically eligible for services.
- TEEN PARENTS WHO LIVE WITH PARENT/GUARDIAN
If you meet the eligibility requirements listed above, please complete this
application. If you have any questions about qualifying, please call an

REQUIRED DOCUMENTS
In order for your application to be processed, you must complete all sections
of this application and include copies of the following documents (documents
will not be returned):
- Birth Certificate
- Proof of Income – Copy of your (and your spouse’s)
current one month’s pay stub or employment verification from your current
employer which states your hourly rate of pay and number of hours worked per
week, or last year’s income tax return /W-2 Form. If applicable, copy of
supplemental security income benefits, disability income, unemployment
income, workman’s compensation income, and or child support verification.
If you are requesting full day services, a validated school/training
verification form must be submitted (full time-12 credits or more), a pay
stub or letter that you are working full time.
- Immunization Records
- TB Assessment and / or test if at risk
- Current copy of IEP/IFSP – if applicable
- Proof of legal custody for children in foster care
- Notice of Action for CalWORKs families
